- Hospital or family to submit an application to Agency for Integrated Care) AIC
- AIC will channel the application to Nursing Home
- Patient will be assess prior admission
- Medical Social Worker (MSW) will collect relevant documents of patient.
- Intake interview with patient’s family
- Acceptance or Rejection of intake of patient updated to AIC, Hospital, and family
